Student Enrollment Agreement

Upon acceptance for admissions at United Tribes Technical College (UTTC), Bismarck, North Dakota,
I, _________________________________________understand and agree to the following terms and conditions:
- My purpose in enrolling at UTTC is to broaden my educational background and learn career and technical skills. With this purpose in mind, I will attend all academic classes/clinicals/workshops/practicums as required and scheduled.
- I will apply myself to the course(s) offered to reach my educational goals, to maintain good standards of conduct, and to respect the rights of persons and property.
- I will consult with my academic advisor and counselor at UTTC before leaving school for any absences.
- I will abide by all UTTC rules and regulations which are stated in the UTTC Student Rights and Responsibilities handbook. This book will be available at the bookstore. It is my responsibility to obtain this book and read it. I understand there are consequences if I do not adhere and follow the college’s rules outlined in the handbook.
- UTTC will provide wellness services to all students and staff within the limits of the existing contract with the Indian Health Service. Health services will not be provided for preexisting conditions or unauthorized medical emergencies. Students must understand that the UTTC wellness program does not cover pregnancies. Therefore, if a pregnancy exists, the student must seek alternate funds to cover medical costs prior to enrollment.
- I understand that UTTC has a zero tolerance policy for illegal drugs and alcohol. I agree to abide by this policy. I agree to refrain from the use of any possession of illegal drugs and alcohol while on campus at all times. I understand that I can face disciplinary action, including suspension and termination from UTTC if I violate this policy.
- UTTC will make available support services such as career counseling, tutoring, disability and any other services that UTTC has available to help me with my education and training and will enable me to accomplish my educational goals.
- In the event that I have children enrolled at Theodore Jamerson Elementary School or with childhood services, I will abide by agreements stipulated or offered by the school or the services office.
- I recognize the UTTC Career Development will provide me with assistance but cannot guarantee a job at graduation.
- After successful completion of the degree plan, UTTC will award me a certificate or diploma when all Academic and Financial obligations are fulfilled by me. I understand that official transcripts or graduation documentation cannot be released until these obligations have been taken care of.
- By signing this agreement, I agree I have read this document and acknowledge its terms and conditions as related to my enrollment at UTTC.
